Maybe you can just type in what you see in the reports. There should be a section called Impressions or Comments or Findings Something along that line). That makes it a bit easier for all of us.

If it is muscle invasive (which it sounds like they already told you), the international gold standard of treatment is removal. Depending on the the depth of invasion, there may be chemotherapy before or after the surgery.

What hospital will she be going to for treatment? The Apollo hospitals are some of the best in India for cancer treatment but it is important to find the top bladder cancer specialists who perform the surgery on a very frequent basis. I haven't been to the others but the one in Bangalore and in Chennai are excellent. India is a big country. What major city is your aunt near?

Johns Hopkins also does second opinions on pathology

but India has some of the finest surgeon training in the world. I just do not know the protocol to get a second opinion there.

Hi Sangeet,

In the US, bladder cancer treatment recommendations are usually based on the pathologic stage, determined by biopsy or turbt (transurethral resection of bladder tumor). Perhaps, the most important feature is the presence or absence of invasion of the muscle wall of the bladder (may be termed muscle wall, detrussor, or muscularis propria). In cases where the muscle wall is involved the most common recommendation is for surgery. I agree that this is complex surgery best performed by a surgeon with extensive experience. Some urologic oncologists in the US, notably at Cleveland Clinic are willing to do consultations based on information provided to them by mail or internet communication.

Sangeet,
its difficult to know how to recommend..but i do know my surgeon who is based here and head of Urology at USC/Norris in California was a fellow at All India Institute of Medical Services in Delhi. Dr. Gupta is head of urology there. He has an email and perhaps you can solicit a second opinion from them?

You definately want a place that sees a lot of bladder cancer and has a specialist in the field. I wish i could be more helpful.

your aunt is young enough to undergo this surgery and reconstruction.
