Spoke to my URO and based on what I was saying on the phone, he immediately prescribed Cipro and Phenazopyridine and wanted me to get on them ASAP.

I have actually found that the Phena does minimize the pain and buring alittle, but it isnt working like I thought it would. I am still getting pretty significant pain when urinating...and the Cipro - boy, that is a ball of fun. Hint to you all - dont take Cipro on an empty stomach - yikes. Threw up all over myself on the drive to work this morning (I normally dont eat breakfast, will be eating it in the future)...that is one strong antibiotic. He gave me 7 days worth - is that enough and how fast does Cipro work? Should I start to feel relief immediately for my UTI or does it take the entire week to usuall work?

Nope, no Mitomycin C that I am aware of...and I think my URO would have told me if he administered it during my surgery...heck, I need to ask him about that. But I seriously doubt it because he was very adament that no further treatment was needed because the tumor was so small and what he thought all along - benign.

Either way, I am going to go in and drop off a urine sample and see if anything turns up.

Dean....Flomax has a pretty long half life so i doubt if you would notice a difference immediately. And as Flomax capsules act by relaxing muscles in the prostate and bladder neck at the site of the obstruction, resulting in improved urine flow and reduced BPH symptoms its a reaction i've never seen or heard of.(which doesn't mean it doesn't happen just not reported). I've heard of all muscles relaxing, sexual dysfunction...but not spasm of the urethra or pain after urination...
Anyway hope you've talked to the doc by now or at least go in drop off a urine sample.

P.S. I can't remember if this is your only thread. Did you say you got a treatment of Mitomycin C intravesical after your TURB/TUIP? Could be a culprit there if you did as
Bladder fibrosis/contraction has been reported with intravesical administration .

BTW, stopping the flomax for 24 hours and seeing improvement isn't necessarily indicative that it was the flomax causing the pain. It could be that you were healing and it is coincidental.

The test would be if the pain comes back if you start back on the flomax. However, I don't recommend doing that without discussion with your uro.
You can certainly tell him what you think you should do and he can advise you on the pros and cons. Since you already seeing improvment in pressure, that is relevant information for your uro to know in making the decision.

Just a follow up since I forgot to answer the part about TUIP getting you off of flomax.

As I understand it, the TUIP takes a while after the surgery for it to give the desired result. That is why flomax is still often continues for some period of time. I don't recall but I thought was a number of months before the surgery itself gave the full benefit. Sounded like you got some immediate improvement so that's a good thing.

My TUIP was done at the same time as a TURBT that determined that my CIS had become muscle invasive so it was only a matter of weeks before I wound up having the bladder and prostate removed. Because it happened so fast, I didn't really see any real improvement from the TUIP.

I had previously had a procedure scheduled for a TUMT (using microwaves with a small rod inserted through the urethra into the prostate area) to shrink back the prostate. Since that was around the same time that I had positive cytologies, we dumped the TUMT idea (with my regular uro) and went forward with my cancer center uro specialist doing the TUIP procedure at the same time as the bladder biopsies.

Your uro can give you and idea how long he thinks it will be before he advises stopping the flomax. It may make sense to be off it for a week or so to let things heal up better and then start back on it for a couple of months or it could be that he decides it would be ok to stop it now for good. He's the guy that should make that call in concert with you.

Dean,

I was on flomax after my TUIP also. While it didn't happen to me, it certainly sounds possible that the flomax could be a contributor.

Since my TUIP, I wound up having to have a full cystectomy/prostatectomy so I quit the flomax. No need if no prostate!

My family doc was pretty darn happy because I gave him my left over flomax. I knew he took it also and I had just gotten a 3 month refill for 2 flomax a day so I just gave it to him.
