For the record we have a few pages on the subject of "surviving hospitalization", mostly geared towards cystectomy patients, and mostly created by them as well!

I do basically the samething Karen with Penn and when I am in there for a period of time I must say my birthday at least 1000 times lol. So I change it for them Day-Month-YR Month-Day-YR YR-Month-Day just to have some fun. Joe

And it still takes them a day to get it straight and they have my list which you know the Dr. sees and they still say waiting for Dr. orders. No place like home. This is going in usually when I leave my meds are the same but my wife being a nurse always checks. Good information Karen. Joe ;)

As Julie correctly addressed in another post she makes sure to have a Medication List when her husband will be hospitalized. I also do this, and this is the format of the list I use.

IN AN EMERGENCY CALL:
Phone #'s

Give a brief description about you/your loved one, or any concerns you have such as
"Risk for Falling", "Weakness in Right Arm" etc...

Have the nurse put it in their chart at admission. Also post another in the patients room.

Be sure upon discharge that the patient is on the correct drugs/dosages...Don't sign anything until it is corrected in the computer...

Some of these things may seem like common sense...Don't assume so...
