  • question on cysto and diagnosis

hello, and thanks in advance for any help!

    my question is….

I had a day and half with heavy bleeding and clots in my urine (male, 40).

I had the ct scan and x-rays, which showed a lump in the bladder.

followed up with the cysto exam the other day. (as well as urine and blood test)

my dr. tells me after the exam that its a tumor, and its cancer.

can they really tell that’s its cancer without the biopsy being done though?

i’ll probably call back today to check with them, but he basically said that when they remove it (scheduled for that in 3 weeks) and test it will be confirmed them, but he says its cancer at the moment?

    does that make sense?

edit: he said its a T1 stage based on what he saw.

Your uro is right on the money with his treatment plan. BCG is NOT as effective with low grade tumors. AND you will want to save the “big guns” in case you need them down the road.

    Congrats on the diagnosis!!!

thanks for the help and info out here, figured I’d toss out an update…

I had the tumor removed on 12/4, urologist was pretty happy with the results.

confirmed low grade T1, early stage.

he decided follow up treatment wasn’t needed, just the standard 3 month check ups for a year, followed by twice a year, followed by once a year.

in a way I guess I am glad he decided that BCG or Mitomycin wasn’t needed…. nut then gain I had that better safe that sorry thought too….

    Hello
And I’m sorry to see you here. But welcome to the club.
I had an ultrasound first and then, before anything else, I was sent to a urologist who walked into the small exam room with this opening line, “So…. you have cancer!”
I guess they don’t sit you down and break it to you slowly anymore.

But, as they watch the cystoscope video, they can tell by the shape of the tumor if it is cancer or not. So it is usually a good guess on their part.

After the TURB confirms this, and they remove the tumor, they will do another TURB to see if there are anyremaining cancer cells in the tissue.

And then you will be put on a course of recovery. Hopefully you will be able to have BCG treatments that will save you from major bladder removal surgery.
But even if you have to go the removal route, you will still be OK. I had mine taken out seven years ago and I’m still here, and living fine.

    Best of luck to you

    George

    very true.

I just feel ‘weird’… close family and a friend or two know about the bloody urine and clot and that I had the cysto done to ‘check’ the lump in the ct scan…..

but I almost feel like, when they ask how it went and what they found, that using the ‘C’ word at the moment isn’t right…. that they might remove it and have it be nothing and then i’ll have to explain why I said it was ‘C’ and when (if) it turns out to be nothing after the pathologists do their thing…. even though I know what the urologist is saying……

just a weird spot to be in and I cant wait until it gets removed and they know for sure exactly what is going… TURB is on 12/4….. three long weeks……

I do have to say, glad I found this place, a lot of very very good (and understandable) info here!

    Hi,
I know just how you feel. Last March my urologist told me the same thing before my first TURB. She turned out to be right and sent me on to a urology oncologist. You will know much more after the pathologist report.

I wish you the best, but keep in mind that knowledge is power and once you know more, you will have options.

    Thinking of you.

    thank you for the info!

that pretty much confirms what I thought, I think I just started confusing myself a bit with everything that he did (and did not say)

they already scheduled the removal of the tumor (i had to ask what the difference was between a tumor and cancer, and he said what I have is a cancerous tumor) which I think was confusing me a bit… he kept calling it both.

and they already have treatment planned.

I guess the more I thought about it, the more surprised I was that he could be so definitive, but I guess in reality its not all that definitive as there are still things to be answered with the biopsy when they take it out.

Yes, it makes sense. While the definitive diagnosis cannot be make under after the biopsy is reviewed by the pathologist, the urologist has had a lot of experience and is making his best “guestimate.” And, he could still be wrong.
    They are, occasionally!!

It is only after the biopsy report comes back that you, and the urologist, will know exactly what, if anything, you might be facing. The pathologist’s report will tell you for sure whether or not it is cancer, the grade (high or low….which indicates how fast it is growing), and whether or not it is still localized in the bladder lining or has extended into the muscle.

At that point you and the urologist will discuss your options. It is important to remember that in most cases bladder cancer is very treatable.

If you have any questions, someone where will certainly try to answer them.
Please let us know what happens…
