night time leakage
Posted by kenneth_ashley on February 25, 2006 at 3:27 pmI am a 65 year old male diagnosed with Bladder cancer… I had a RC at the end of October 2005 and a neuro bladder constructed.. every thing went fine and I am going through a speedy recovery… I have no leakage at all durring the day …but on the other side I have no control at all durring the night.. the depends just doesn,t help and I get up soaked at least three times a night… have any of you out there had any experience with other methods.. such as the male penile clamp.. or any other device.. I do my Kegle exercises but they do not help either… thanks Ken

Noticed that this thread hasn’t “concluded”. How are things going? Have you reached the
night time continence club? Last post was 2/17/06. One comment that caught my eye was
the statement about going 5 hours without “going”. Hope by now you realize its not time so
much as amount that’s important. If you’re going 5 hours and voiding in excess of 500cc that
ain’t so good. 350cc to 500cc seems to be the standard maximum goal.Let us know how things are going. Hopefully, the goal has been achieved.

neobladder 9/4/04Thanks .. I am doing everything you suggested already.. have no problem durring the day sometimes I hold it for 5 hours,.. but no control durring the night .. wearing double depends and getting up soaked at least three times a night … Have you heard of the clamp which is supposed to help in the evening ? and how long before you gained nighttime control… Ken

Thought I’d chime in on night time continence. I had my neobladder made in May of 2004. While daytime continence happened a few weeks after catheter removal, night time took a few months. Before I get to a few things that might help, I wanted to mention a few things to think about.
First, have you gotten to the point where you can hold 400 – 500 cc’s during the day without leaking? This is important to increase the capacity of Mr. Neo so that there is less tension, and less likelihood of leakage – especially during the night. If you haven’t gotten to that point, Its important to do so by gradually increasing the time interval between voidings. That way, your bladder won’t be trying to force urine out at low volumes and you’ll be less likely to leak at night when you have less muscle control.
In the mean time, there are a few things that might help. First, try wearing a depends guard for men – one or two in addition to the brief. I’m a side sleeper so I wore one sideways for a while, which seemed to catch better than the conventional position. I also put a small 18″ x 24″ waterproof pad under my midsection to catch the inevitable leaks. There’s no use adding any stress – I simply changed the pad and replaced it with another one so I could get back to sleep.
I found that salt intake affected water retention, and when the water wanted out, it wanted out, and that was usually at night. I reduced salt intake and that helped a good bit. Emptying thoroughly before bedtime helped me (sitting works best for some people). I noticed eating a small snack usually reduced output – you might try that before bedtime if your diet allows. Oddly enough, you might want to increase water intake during the day to thin out the mucous – sometimes this can get on the depends pad or guard and prevent absorption. And limit fluid intake in the evening. You might try getting up twice a night to empty until you can gradually change to once a night – I set the alarm clock across the room to make sure I get up – otherwise I’d just turn it off and go back to sleep. Hope this helps – if I think of anything else I’ll Post it. Try not to get discouraged – night time continence can take several months to happen. Its encouraging that you’ve achieved it during the day – hopefully things will improve at night.

My husband had a neobladder Mar 05. For months he had the same problem…never getting better. He tried alarms, external catheters, no liquids after 6:00 PM and getting up 3-4 times a night. He still soaked the bed, pads, his t-shirt and even the pillow sometimes.
About 2 months ago, this was discussed again with his urologist. The Dr. did a “doppler” on the bladder and found that it was not emptying completely when urinating. My husband was instructed on catherizing every night prior to going to bed. THANKFULLY this stopped most of the nite time problems. He still has “leakage” that is usually handled with a depends (still uses one pad just in case) . One thing I found helpful: I bought King Size waterproof mattress covers, cut them into 6 pads, sewed the edges. This provided much larger pads than can be bought and much less expensive!
