  • Cunningham Clamp

    I’ve been directed by my doctor to use a Cunningham clamp for some period of time (I believe it is to be about a month). This is to keep me dry at night (daytime is okay) and to keep my new neobladder stretched from the buildup of urine.
    Does anyone have any experience with this product?
    (I have found it uncomfortable, painful at times and I still leak periodically.)

    Thanks. Agreed…almost there.
    Day time at home…I tend to go without pads.
    Day time out however, I wear a pad for protection from “dribbles”.
    Evenings are another story.

    PJF,

    I haven’t heard anyone else using clamping devices or the like for night time. Most don’t use anything at all except for wearing a Depends-type undergarment and setting the alarm to wake up.

    For the condom catheter, I am only suggesting that as a means to staying dry when you leak at night. You should not just use it to “let go” at night. You still get up and go according to the schedule.

    Talk to your uro about it but some of us don’t think clamping is a very good idea at all. As already mentioned, it can cause complications that you don’t want and if you are already having discomfort, that sure seem like a sign.

    You are still stretching the bladder during the day with your schedule. Same thing by waking up and going at night. It’s not going to shrink up by going nights according to your same schedule but then leaking some.
    If your uro thinks otherwise let me know and I’ll bet I can find about 100 other neobladder surgeons at top bladder cancer centers that will go kick his butt and bury his office with the research papers that say otherwise.

    Thanks for your reply.
    I’m 2 months past surgery, doing kegels regularly, and discussed condom catheters. But at this stage, surgeon wants to keep the neo filling up so we “train it” to maintain a capacity level. I can certainly relate to the replies from you and Mike….with a smile, of course. I was surprised when the term “Cunningham” was no where to be found on this website. I need to ask some more questions of my surgeon.

    Yikes!!! How about a big rubber band???!!!

    To keep dry at night consider a self adhesive condom catheter.
    I don’t use the kind that have that wrap around band because they can cause problems with blood flow.

    Clamps and the like pinch off blood flow and as Pat already mentioned can cause other problems. Not worth the risk. Ask your doctor about using a condom catheter at night and still sticking to your wake schedule to drain the neo.

    You hook the condom catheter to a catheter bag at night. Don’t try to get creative with it and close off the tip with something or you discover that the pressure of the urine will cause it to expand and leak all over you and the bed.

    Geez..i thought that went out with torture chambers?
    Penile clamps, indwelling catheters, condom catheters and pads are occasionally used but are not generally considered socially acceptable and can be a source of anxiety and discomfort if incorrectly fitted. They should be reserved for minor degrees of incontinence or in patients who have multiple other co-morbidities in whom surgery may be thought inappropriate.

    The safety, efficacy, comfort and patient satisfaction with three types of penile compression devices (C3, U-Tex Male Adjustable Tension Band and Cunningham clamp) was evaluated in a small (12 patients) randomized control cross-over trial. The Cunningham device was the most efficacious and acceptable to users, but, significantly reduced the distal blood flow velocity. None of the devices completely eliminated urine loss when applied at a comfortable pressure. Potential complications of penile clamps include edema, pain, urethral erosion and obstruction and they should not be used for more than four hours at a time.
    How long have your been out of surgery? Do you do the kegals? Mike has a pretty good idea on the condom catheter for night.
    And if you have a really good surgeon he can implant a sphincter valve.
    Just my opinion…….OUCH :S
