2nd TURB
Just had my 2nd TURB done yesterday and this time the surgeon went into the muscle and he did remove a tumor. At this point that is all we know because the pager that my family had apparently didn`t work so after 30 mins of trying to get them, the surgeon left to go to another hospital. He is supposed to call tomorrow and talk to us.
So, my question is this. If the first TURB path report was malignant inside the bladder, does that automatically mean this muscle tumor is as well? Is it possible to have both types of tumors in the bladder? -

A definition of pT1 “The tumor has spread to the connective tissue (called the lamina propria) that separates the lining of the bladder from the muscles beneath, but it does not involve the bladder wall muscle.”
The consensus of how to treat this is: There is no consensus.
What this means is that some urologists recommend immediate cystectomy (removal of the bladder) and others prefer trying treatment with BCG. There are risks with both as mentioned in all the articles I have read: for many patients radical cystectomy may be “over-treatment” and not necessary while for many others BCG does not work and only delays the necessary surgery putting the patient at risk for further spread. You can GOOGLE pt1 bladder cancer and read many articles that discuss this without coming to any conclusion as to how to determine which patients can benefit from BCG and which need immediate cystectomy.
You need to listen to your doctor’s recommendations carefully and most likely seek a second opinion at a major bladder cancer center. This is something that you need to consider very carefully and only you, and your doctors, can decide what is the best thing in your specific case.

Haven`t spoken to the MD yet but was able to see the path report:
Both pre op and post op dx: Malignant neoplasm of trigone of urinary bladder
Final Pathological Diagnosis
Deep Bladder Resection
Invasive High Grade Papillary Urothelial Carcinoma
Carcinoma invades the Lamina Propria/Subepithelium (pT1)
Muscularis Propria ( Detrusor Muscle ) present and free of CarcinomaCan someone please tell me what the pT1 means? Any info to ease my mind would be helpful and much appreciated.
The good news is that it isn`t in the muscle, right?
I have learned we simply must wait for pathology. The surgeon went into the muscle for “margins” and the path report will tell whether it has stayed outside or has penetrated it. All proper procedure. Anything beyond that is simply conjecture. I know the wait is tough but, it is “one day at a time”.
The good news is you will have a plan of attack soon. We are all with you and post once you know anything, someone will listen.
DX 5/6/2008 TAG3 papillary tumor .5 CM in size. 2 TURBS followed by 6 instillations of BCG weekly with a second round of 6 after a 6 week wait.Sign In to reply.
