I received the written urine cytology report today. It has no detail as to why the conclusion of Urothelial carcinoma, high grade was diagosed. I wrote to the pathologist asking for more detail. Pathology reports certainly vary according to the institution and pathologists. I have been given some very detailed reports by other institutions. This one at VCU is always very brief with no details, just a diagnosis. Not very satisfying.

Ro, No I have never had a positve urine cytology before this one. I have not as yet received the written report though I have been awaiting it. I did learn that another blc lady had a positive urine cytology report but when the tumor was sent to pathology it was a TA G1. HAS ANY ONE ELSE had a positve urine cytology but that was then analyzed after pathology as low grade?

Wow! Thanks Wendy and Dr. O'Donnell. I am the person who always encourages others to get written reports but I have never seen nor known there was a detailed report for urine cystology. I am being even more educated (and hopefully helping to educate others through my expereince) after 6 1/2 years. I shall ask my doctor to mail me a copy of the the urine cytology report. Thanks so much for your invovlement Wendy. You always seem to be my special guardian angel.

Well Rosie, we got a reply from Dr. O'Donnell already, we're lucky~

"I’d look at the details of the cytology report. If it shows “papillary clusters of cells” consistent with papillary TCC then no problem. Even a blind pig can find a truffle occasionally. If, however, there are severely atypical or highly dysplastic cells with high nuclear to cytoplasm ratios consistent with high grade TCC then she needs cystoscopy with random biopsy and upper tract washes to look for a NEW high grade lesion or CIS. Drugs and supplements do NOT cause these changes."

That makes sense.

Dear Rosie,

I don't know the answer but would like to...I've sent the question off to an expert and hope he replies, I"ll let you know.

I'd say do a repeat cytology if the doctor is willing, and if it comes up positive again you're in for a TUR and biopsies. I know the risk of sudden jumps in grades and stage is low, but I also know women who have gone from Ta, G1 to having CIS and even mets quite unexpectedly, after ten+ yrs of conservative treatments.

I know you're careful, and that you know what you are doing. I hope this turns out to be nothing.
