Sorry you have to be here but welcome Nell. It sounds like you know your questions you just have to sit down and list them. In some situations someone can go into a rehab center after leaving the hospital. Talk to your medical team about getting a visiting nurse if needed. We got a CNA to come to the house for eight hours a day to help out for a few weeks. It was expensive but really was a massive help. There are ways talk to your medical team and make a plan. As far as infections and care instructions they will tell you what you need to look for and what to do. I know this is very overwhelming right now I know I have been there but we all find a way and make it though. And some day you will look back and it will be a memory for the vast majority of us life goes back to normal and we learn to live with our new plumbing. We are here for you I am sure they others will have more to add.
