Thought I'd chime in on night time continence. I had my neobladder made in May of 2004. While daytime continence happened a few weeks after catheter removal, night time took a few months. Before I get to a few things that might help, I wanted to mention a few things to think about.
First, have you gotten to the point where you can hold 400 - 500 cc's during the day without leaking? This is important to increase the capacity of Mr. Neo so that there is less tension, and less likelihood of leakage - especially during the night. If you haven't gotten to that point, Its important to do so by gradually increasing the time interval between voidings. That way, your bladder won't be trying to force urine out at low volumes and you'll be less likely to leak at night when you have less muscle control.
In the mean time, there are a few things that might help. First, try wearing a depends guard for men - one or two in addition to the brief. I'm a side sleeper so I wore one sideways for a while, which seemed to catch better than the conventional position. I also put a small 18" x 24" waterproof pad under my midsection to catch the inevitable leaks. There's no use adding any stress - I simply changed the pad and replaced it with another one so I could get back to sleep.
I found that salt intake affected water retention, and when the water wanted out, it wanted out, and that was usually at night. I reduced salt intake and that helped a good bit. Emptying thoroughly before bedtime helped me (sitting works best for some people). I noticed eating a small snack usually reduced output - you might try that before bedtime if your diet allows. Oddly enough, you might want to increase water intake during the day to thin out the mucous - sometimes this can get on the depends pad or guard and prevent absorption. And limit fluid intake in the evening. You might try getting up twice a night to empty until you can gradually change to once a night - I set the alarm clock across the room to make sure I get up - otherwise I'd just turn it off and go back to sleep. Hope this helps - if I think of anything else I'll Post it. Try not to get discouraged - night time continence can take several months to happen. Its encouraging that you've achieved it during the day - hopefully things will improve at night.
