Welcome. We are sorry for the reason to be here and the difficult report on your husband. As you are aware stage 4 of any type of cancer is very serious when metastatic. For BCG it is a local treatment (confined to the bladder) which would do nothing to help with lymph nodes involvement.

Everyone is an individual and a few react well to chemo. No one seems to have a handle on why. Perhaps a second opinion would be good for fresh eyes and a top notch teaching or major cancer center such as MD Anderson, Cleveland Clinic, Sloan Kettering etc. Or, perhaps your local team may have access to a clinical trial that may have some promise.

DX 5/6/2008 TAG3 papillary tumor .5 CM in size. 2 TURBS followed by 6 instillations of BCG weekly with a second round of 6 after a 6 week wait.

My husband was diagnosed with stage 4 metastatic urothelial carcinoma in August, 2019. Because it was found in several lymph nodes they decided to not do bladder removal. He had previously undergone a cystometry to diagnosis that tumors were found in his bladder as well as a TURBT proceedure to remove the several tumors. We anticipated having him do the BCG treatment. But they decided to biopsy lymph nodes and found cancer in them so no BCG and no bladder removal. He is about to begin cycle three of chemo (cisplatin/gemcitabine) he also needed to have a Nephrostomy catheter placed in his left kidney due to high creatinine levels. My question is does anyone know why they would not consider a chance for a cure? Which is what we have been told over and over. That there is no cure for bladder cancer. That the most we can hope for is to stop it from growing.
