That zinger I presume is from the TURB and everyone is different. Depends on where and how much digging/burning is done. I had mine early one day and was at work the next day.

I am still confused on bladder removal and construction comments. If this is NOT muscle invasive then BCG is the normal path and NOT bladder removal. Plus no radiation for sure. The next scope I am sure is to make sure the TURB work is clear and then you should go to BCG.

DX 5/6/2008 TAG3 papillary tumor .5 CM in size. 2 TURBS followed by 6 instillations of BCG weekly with a second round of 6 after a 6 week wait.

Met with the oncologist today and was told that no treatment would be done until I had another bladder scope done to see if anything had come back. The surgeon had us thinking and stressing for nothing at this point. The oncologist said he will be sending me to a different urologist that specializes in bladder cancer and surgery ( removal, reconstruction ) He prescribed Tramadol for the God awful " zingers" of sharp pain I am having.

So here`s a question for you... Is the sharp "zinger" pain ever going to stop? I have to watch what kind of surface I sit on and how I sit. I get them when I take a step to walk, hitting a bump while driving, coughing. The pressure is gone when I urinate but again the sharp " zinger" hits me, sometimes very hard taking my breath away.

Thank you Alan for that info. I feel so much better now knowing what stage it is and that he did do a sample of the muscle. Will know more tomorrow after oncology visit.

Yes, non muscle is stage 1. The usual protocol is BCG/immunotherapy. One current problem. There is a national shortage on this so press your team to get started. Other agents in lieu of this includes some types of chemo as a wash inside the bladder, NOT systemically.

The surgeons nurse just called and told me that no cancer showed up in the muscle and the tumor was a T1? Is this the same as stage1?

Well, that answers a few questions and raises more! It is high grade, meaning that the cells are very abnormal in appearance and are rapidly dividing. High grade has the potential to continue to spread. It has invaded the lamina propia, or the first layer of the bladder lining.......it states that this invasion is a "focal invasion." Does this mean that it has spread only at a small location? And next is the critical part....no muscle tissue was in the sample. I would want to know how the urologist knows whether the tumor has spread into the muscle or not.

IF the tumor has not spread into the muscle and IF the invasion into the lamina propria is not extensive, BCG might be an option. IF it has spread into the muscle, usually this means that radical cystectomy in indicated. Since this is such a critical factor, in many cases a second TURB is done to determine whether the muscle is involved or not.

This should indicate some questions for your doctor.
