Of course your brother-in-law should call his doctor tomorrow and have this checked out. It would be a very unusual symptom for bladder cancer; it is most likely an infection that can be treated with antibiotics. But, this is for his doctor to determine.

My bro in law was dx with bladder cancer after cystoscopy 3 days ago. Surgery not even scheduled yet. His testicle is swollen and painful too, though. Does this mean it has spread? We are scared to death.
CAT scan before cystoscopy showed 2 cm area that was concerning. Blood in urine made him go to emergency room.
