Oh Mike! The picture gives me that "yeeaaaaaoooooooouuuuuuchhh! ' memory.
I remember it being so much less invasive when I didn't know anything!

Urine sample is the same. You pee in a cup. They run different tests on it but it doesn't change your part.

Cystoscope: All depends. May do then and there or may have to schedule you for it. Typically done in the office to have a look around in your bladder.

Probably be a bunch of forms to fill out.
Bring a list of all medications and supplements (if you take any)

They ask you questions about your symptoms, probably have you give a urine sample, etc...

You may get a cystoscopice exam if the uro feels your symptoms warrant it.

Sorry not a guy but the clue here to me is the foamy urine..you should be checked for protein in the urine.
